The first edition was published in 2003; this second edition is substantially revised and updated. Bruera (U. of Texas M.D. Anderson Cancer Center) and Portenoy (Beth Israel Medical Center and Albert Einstein College of Medicine) maintain the goals of supporting better understanding of pain and pain management, and improvement of care and access to care. Their preface emphasizes the continuing global rise of the disease and the "poor assessment, undertreatment, and problems with drug availability...that compromise best practice" in both the developed and developing worlds.
